The civilian-led Aurora Mobile Response Team (AMRT), the Aurora Police Department's Crisis Response Team (CRT), and the Targeted Violence Prevention (TVP) program operate to provide a continuum of care through direct intervention, individualized crisis case management and resource referrals.

The Colorado Coalition for the Homeless seeks an Integrated Health - Behavioral Health Clinician II, Social Worker to provide care that includes assessments, diagnostic evaluations, patient education, crisis intervention and safety/risk assessments, brief and long-term therapeutic interventions, group therapy, care coordination and case management, for individuals with mental/behavioral health and substance use issues.

This experienced Licensed Clinical Social Worker (LCSW) will provide behavioral health services for our underserved patient populations providing crisis intervention, brief and longer term counseling, some case management, triage & linkages to social services, housing linkages and other services to assist in providing comprehensive services to ensure better health and educational outcomes.

Responsibilities The Behavioral Health Clinician works under the supervision of a licensed clinician and provides all aspects of psychosocial mental and behavioral health services including primarily brief short-term individual and group therapy, diagnostic evaluations, assessments, treatment planning, care coordination, and case management.

Responsibilities of the BHS also include the development and provision of therapeutic skills groups in collaboration with Behavioral Health Therapists, completion of clinical documentation of patient progress, contribution to individualized treatment plans, de-escalation, crisis intervention, and actively participating as a member of the interdisciplinary treatment team.

GENERAL STATEMENT OF DUTIES: Under the direction of the Manager of the Behavioral Health Urgent care, performs supervisory and professional tasks in the staff development, supervision, case consultation and management of the program, including but not limited to the provision of Crisis intervention services, intake/biopsychosocial assessment services, therapeutic support, and solution focused interventions on an occasional, intermittent, emergent basis; performs related work as required.

Work involves providing direct services to crime victims to include mandated services, crisis intervention, case management and treatment services, and maintaining effective relationships with stakeholders and other victim-services agencies.

Crisis Intervention - As needed, the Licensed Professional Mental Health Counselor(LPMHC) makes rapid assessments and develops crisis management plans to meet Veteran needs in the least restrictive setting; in the event of a medical or psychiatric emergency, LPMHC will follow the protocol of the Medical Center.

Crisis Intervention - As needed, the Social Worker makes rapid assessments and develops crisis management plans to meet Veteran needs in the least restrictive setting; in the event of a medical or psychiatric emergency, Social Worker will follow the protocol of the Medical Center.

Current and valid State of Florida License in Mental Health Counseling or Licensed Clinical Social Worker license required or Licensed clinical psychologist. Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of medical and psychiatric chronic disease.

This unique opportunity combines elements of direct patient care, providing individual crisis intervention therapy, group facilitation and case management to patients with behavioral health needs.

Utilizes principles of Health Psychology to develop implement, coordinate and evaluate treatment plans for referred members that may include crisis intervention, brief psychotherapy, psychoeducation groups and brief case management.

Licensed Professional Clinical Counselor (California) OR Licensed Clinical Social Worker (California) OR Licensed Marriage and Family Therapist (California) Masters degree in Social Work, Social Welfare from a clinical track, Clinical or Counseling Psychology or related field required from an accredited college or university.

Clinicians not licensed will provide structured chemical dependency and mental health services with a focus in supervision on providing the appropriate delivery of twelve core functions: Screening, intake, orientation, assessment, treatment planning, therapy, case management, crisis intervention, client education, referral, reports, record keeping, and consultation.

Identification - Veterans are identified for the potential need for case management through self-referral, referral from family/caregivers, referral through the Veteran Crisis Line, BHIP, or community partners.
