The issue may involve the care given to a single Veteran, to groups of Veterans at one or more VHA healthcare facilities, or it may involve examination of system wide healthcare practices.
ResponsibilitiesThe incumbent independently provides clinical psychosocial and case management services at an advanced practice level to injured/ill returning combat veterans as well as non-combat veterans who are transitioning off of active duty.
The Incumbent's principal role is that of TCM Case/Care manager providing ongoing care management services to returning combat veterans and non-combat veterans and their families over the course of time VHA health care services are to be provided.
