- While working collaboratively with the clinical operations leadership team, the Head of Nursing is responsible for overseeing and coordinating the nursing staff (Clinic, Population Health, Triage) and nursing clinical operations within a primary care setting.
- Provide managerial oversight of registered nurses and other clinical support staff as delineatedLead quality improvement initiatives including quality, environment of care, infection control, etc.
- Assist in the development and implementation of clinical programs, including, but not limited to: chronic care management (CCM), remote patient monitoring (RPM), chronic disease-specific interventions, intensive care coordination, etc.
- Drive performance to organizational goals and OKRs. Population Health Program Development and Implementation: Collaborate with healthcare providers, community organizations, and other partners to execute these programs.
- Working with the Clinical Service Manager and Nurse Manager will help design, develop, and implement population, health programs and quality measures and interventions to address the specific needs of target populations.
