Registered Nurse-Community Living Center Admission Coordinator

The VA Northeast Ohio Healthcare System is recruiting for a CLC Admissions Coordinator with Nursing/GEC..NOTE: The 2-page Resume requirement does not apply to this occupational series. For more information, refer to Required Documents below.The Registered Nurse-Community Living Center Admission Coordinator duties include but are not limited:Screen and review all referrals for admission to the Community Living Center, including some in person, ensuring timely and appropriate placement based on program criteria.Work closely with healthcare providers, VA and non-VA departments, and community agencies to facilitate seamless transitions for Veterans into the CLC setting.Serve as the primary point of contact for bed management and communicate screening decisions to relevant stakeholders.Conduct systematic data gathering, assessment, and nursing judgement to ensure an appropriate treatment plan can be implemented during their admission to CLC.Participate in creating, evaluating, and revising policies, procedures, and guidelines related to CLC admission and placement processes.Educate Veterans and families about CLC services, advocate for patient rights, and address barriers to admission or care.Gather and analyze data to support program planning, quality improvement, and resource utilization for CLC admissions.Utilize evidence-based practices to improve admission processes and outcomes, sharing results with peers and leadership.Provide constructive feedback, support colleagues, and foster a safe environment for professional development within the nursing team.Adhere to infection control, privacy, and safety standards; maintain compliance with VA policies, federal regulations, and ethical guidelines.VA offers a comprehensive total rewards package: VA Nurse Total RewardsPay: Competitive salary, regular salary increases, potential for performance awardsPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Licensure: 1 full and unrestricted license from any US State or territoryWork Schedule: Monday-Friday; 7:30am-4:00pm with potential rotation to off tours, weekends, and holidays.Telework: Not AvailableVirtual: This is not a virtual positionRelocation/Recruitment Incentives: Not AuthorizedPermanent Change of Station (PCS): Not Authorized
