If you are looking for a rewarding career, we encourage you to apply to join our team at AWARE.Responsibilities:Ability to work independently in assigned location/ regionAbility to travel via personal vehicle or AWARE vehicle to client or service areaExperience leading and instructing participants in gaining life skillsExperience in planning and leading recreational activitiesImplements interventions that are sensitive to the client age, cultural background, stage of development, limitations of disabling conditions, skill and experience level.Role model by demonstrating and engaging in activities alongside client to help promote positive lifestyle skillsAssist TRPD in creating protocols for interventions that include short term goals and criteria for assignmentObserve client response to intervention to verbally communicate and collaborate with TRPD and treatment team membersDocument outcomes of the intervention in EHR per policy to provide evidence for TRPD needed for evaluation of TR program effectiveness.At time of transition or discharge, provides TRPD with client information to assist in formulating a transition/ discharge plan.Excited to join our organization?
