﻿Job Description The Department of Mental Health (DMH) with the New Bedford Site Office is seeking a talented and experienced Transitional Age Youth (TAY) Case Manager (Human Services Coordinator I) to serve as part of the DMH New Bedford Case Management team. The incumbent to this role will provide assessment, outreach, support, service coordination and person-centered advocacy to DMH eligible Transitional Age Youth, caseload at least 75% consisting of persons between the ages 16 to 25, by promoting the agency's philosophy of recovery, person-centered choices and cultural competence. The successful candidate will provide advocacy and collateral work with parent(s)/LAR/school personnel/etc., and coordinate and assess client services and entitlement programs. The selected candidate will conduct person served and family assessments, utilize strength-based goal setting, development, implementation, and monitoring of individual service plans and assisting individuals in securing services while performing all duties to reflect the highest level of respect and dignity for the persons served. Please Note: This position is 40 hours per week. The selected candidate will work Monday through Friday from 8:30am - 5:00pm. The days off are Sunday and Saturday. Duties and Responsibilities(these duties and responsibilities are not all inclusive): • Interviews individuals and others and reviews medical and psychiatric records to assess individual needs, completes a Comprehensive Assessment, recommends treatment, and/or service modalities: including emotional, social, vocational, financial, health, recreational, and legal factors. • Assess person served needs and recommend treatment and/or service modalities, including emotional, social, family, educational/vocational, developmental, financial and legal factors; and develops and writes Individual Service Plans (ISP) which includes CANS and CERF-R, as appropriate. Incorporate a thorough knowledge of age-appropriate developmental issues and include activities surrounding needs of person served. Assessments and ISP's involve clients, family members, natural supports and other providers identified by the youth/parent, as appropriate to person served' s age and wishes. • Monitors the implementation of Individual Service Plans (ISP) required by department regulation as a member of an interdisciplinary team; organizes and schedules client treatment and ISP annual reviews and assesses treatment progress and service delivery; meets with clients and families to coordinate service delivery, to assess treatment results and improve services when needed; and makes referrals to other programs and facilities as needed. • Provide creative engagement strategies including face-to-face meetings with person served, family and other providers to engage and sustain involvement in attaining person served directed goals developed. • Performs other duties such as coordinating urgent interventions; consults with service and treatment providers and community agencies on Child, Youth and Family and Adult Case Management issues; providing and/or arranging transportation of clients; and attends and participates in client discharge planning. • Organize, direct and initiate TAY activities in Site benefiting caseload such as peer mentoring, GIFT trainings, leadership councils, support groups, and community resource development in order to improve service array for TAY and families. • Actively strives to incorporate a stable family connection for TAY via psychoeducation. • Maintains contact with collaterals including school, vocational program and therapist to ensure coordinated care, as appropriate. • Link person served as needed to specialty resources such as for LGBTQ, co-occurring disorder and housing resources. • Actively participates in the pre-admission, treatment and future vision planning for assigned individuals in continuing care units, IRTP's, psychiatric hospitalizations and other out-of-home placements, as needed. • Coordinate, monitor and assess the quality and quantity of services for Case Management reviews; coordinate, facilitate and document annual and periodic ISP reviews as required by department regulations. • Assists person served and families in advocating for appropriate academic and vocational services in developing educational plans and IEP's for clients. • Assists in developing appropriate policies and procedures through collaboration with administrators and supervisors. • Prepare and maintain case records and narrative, statistical and team meeting reports, and other documentation. • Maintain MIS standards, including MHIS and PDI entries and updates in electronic and written records, per site and area protocols. • Participate in utilization review for contracted services for person served to ensure person served is in least restrictive service. • Participates in inter-agency treatment planning on assigned cases and prospective referrals. Required Qualifications: • Knowledge of psychosocial problems of those with mental/emotional disorders and the multiple handicapped. • Knowledge of the developmental and cultural needs of the Transitional Age Youth. • Demonstrated skill in the principles, practices and techniques of counseling, interviewing, motivation and reinforcement. • Experience with the types and symptoms of mental/emotional disorders and the programs and services available to this population. • Expertise in the methods of general report writing. • Knowledge of principles and practices of crisis intervention and group process for counseling the mentally ill. • Ability to prioritize and multi-task duties based on client need, team and service needs.
