- S/he is responsible for identifying and accessing resources beneficial to residents as needed, act as a liaison between the Housing Authority/Property Management and residents when appropriate and work collaboratively with Housing Authority/Property Management staff on behalf of all residents.
- Regularly educate residents on the components of the Supportive Housing Program (24/7 emergency response, congregate meals, connection to resources, etc.)
- Conduct field visits to assess an older adult's physical, social, emotional, and environmental status to determine their eligibility and needs in their homes in accordance with Executive Office of Elder Affairs regulations.
- Provide accurate and timely documentation to meet all contractual and administrative standards as required by the Executive Office of Elder Affairs, AgeSpan, and, if appropriate, the Housing Authority or Property Management.
- Provide regular updates, feedback, and reports to Housing Authority or Property Management staff to preserve residents' tenancy.
