- Actalent is looking for a full time 100% remote RN Case Manager, must have CA RN License and working Monday-Friday 8am-5pm PST, making $38/hr.
- Schedule: Monday-Friday, 8am-5pm PST (maybe 7am start date if needed) Fully remote, must have CA license
- Position Purpose : Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care.
- Training: 3-4 weeks, and assigned 5-10 referrals daily, Case management experience is required, will be working 4 hours/daily on outbound phone calls reaching out to members , *Calls are recorded*, audio audits, Case load requirement is 75 active cases, cases are 90 days, using True Care medical records system
- MUST HAVE: Medicare / Medical, case management experience- Preferred: psych/behavioral health background , bilingual in Spanish
