Overview Works as a member of an interdisciplinary care team at a NORC site location and is responsible for the integration of health focused information, education, assessment, advocacy, and linkage to care for residents who live within the boundaries of the NORC program. Provides interventions designed to engage residents in self-care management health strategies that support successful community living. Works under general direction. Compensation: $40.95 - $51.19 HourlyWhat You Will Do<ul><li>Collaborates with the NORC site staff to ensure residents engage in self-care management health strategies.</li><li>Provides health focused outreach to seniors who may benefit from supportive services provided by the program.</li><li>Implements health promotion activities such as health screening, health assessments, health care linkages, health care referrals and health presentations.</li><li>Makes telephone calls and home visits to individual residents aimed at assessing their needs and supporting successful community living activities.</li><li>Makes referrals to appropriate health services based on the assessed needs of the resident.</li><li>Provides Health Care Advocacy by acting as a liaison between the various healthcare systems during times of transition.</li><li>Participates in the Quality Assurance Program and Education Program as requested by the Clinical Manager.</li><li>Refers residents needing direct patient care to their physician or to appropriate health care services to address their particular needs.</li><li>Documents all nursing interventions provided as required by site location.</li><li>Participates in special projects and performs other duties as assigned.</li></ul> Qualifications <ul><li>License and current registration to practice as a Registered Professional Nurse in New York State required</li><li>CPR/BLS Certification required</li><li>Associate's Degree in nursing required</li><li>Bachelor's Degree in nursing preferred</li><li>Minimum of two year of experience working as an RN in LHCSA or CHHA. required</li><li>Prior experience working with an aging population and providing clinical assessments of their health needs. required</li></ul> Associated topics: cardiothoracic, care unit, hospice, intensive care, intensive care unit, mhb, neonatal, nurse rn, psychatric, transitional
