- Provides nutrition counseling, nutrition education, referrals to healthcare and social services, breastfeeding promotion and support, and WIC certification services.
- Provides nutrition education in a group or individual counseling in the areas of infant, childhood, prenatal, and post-natal including breastfeeding, nutrition, and physical activity to promote development and maintenance of good nutrition and lifestyle habits.
- Develop, monitor, evaluate, and revise nutrition education plans for individual low risk and high-risk WIC program clients, not at medical nutritional risk.
- Documents in FL- Wise data system using SOAP format and other documentation guidelines; monitor accuracy of growth grids; distribute exempt formula to clients based on a valid medical condition and prescribed by a Physician/APRN/Physician Assistant.
- Participates in the development, planning and carrying out of objectives/activities in the Bi-ennial Nutrition Program Plan. Coordinate nutrition education services for assigned clients and makes referrals for appropriate services.
