- The LCSW functions as a core member of a collaborative care team that involves a psychiatrist, CRNP DNP and other behavioral health providers in an outpatient setting.
- Assess or provide therapy, counseling, intervention and/or education to individuals with primarily IDD/Mental Health, anxiety, depression diagnosis in a professional manner.
- Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant or CRNP-DNP and communicate resulting treatment recommendations to the patient's primary care provider, if needed.
- Document patient progress and treatment recommendations in the registry so they can be easily shared with the psychiatrist, CRNP-DNP and other treating providers.
- Facilitate referrals for clinically indicated services outside the outpatient clinic setting (e.g., social services such as but not limited to vocational rehabilitation, mental health specialty care, substance abuse treatment).
