- I-CORT Peer Support Specialist
- Provides services to persons with severe and persistent mental illness who qualify for services under the Intensive Community Outreach and Recovery Team (I-CORT) program and serves as a member of the I-CORT multidisciplinary team.
- Provides support to individuals, both on a planned and as-needed basis, to help them accomplish their personal goals, to cope with the stressors of day-to-day living, and to reach goals as identified on the ISP and Recovery Support Plan.
- Provides one-on-one and family support to develop problem solving and coping techniques, empowerment and self-advocacy skills, and identification of natural/community support systems.
- Assists individuals by modeling effective coping techniques and self-help strategies based on prior experience, supporting individuals in advocating for themselves to obtain effective services, and helping them to monitor their progress.
