- The Housing Specialist provides supportive services including outreach, monitoring, assessment and service planning, support network building, consultation to families, side-by-side skill training and instruction in the use of community resources, including referrals to community assistance as needed.
- Provides service coordination for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team (ITT); assumes responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals in collaboration with the client and team; provide individual and supportive therapy and symptom management.
- Assists clients to find and maintain a safe and affordable place to live – apartment hunting, finding a roommate, landlord negotiation, cleaning, furnishing and decorating, and procuring necessities.
- Provides nutrition education and assistance with meal planning, grocery shopping, and food preparation.
- Provides on-call crisis interventions and serve as back up after hours on rotating schedule.
