- The SF HOME Case Manager/Section 8 Housing Locator provides supportive case management services for a caseload of 15-18 homeless and low-income families to help them obtain and maintain permanent housing and stability.
- The SF HOME Case Manager/Section 8 housing Locator conducts home and field visits in the community.
- Maintain Communication and coordination with Coordinated Entry Access Points, The Housing Authority, and EHV case management partners to remove any barriers to the housing referral process;
- Services provided include, but are not limited to, housing search assistance, crisis intervention, community and support services, information and referral, family budget assistance, job enhancement and support, and landlord advocacy.
- BA degree and one year of experience preferred or AA degree and three years related experience.
