- The Homeless Outreach Specialist (HOS) will provide outreach services to individuals who are homeless or at imminent risk of being homeless and who MAY have Serious Mental Illness (SMI) or Severe and Persistent Mental Illness (SPMI) and co-occurring substance use disorder.
- Homeless Outreach services includes contacts to assist with housing and other basic needs, including referrals to appropriate screening and evaluation and/or treatment for mental health needs and functional support services.
- This role will report to the Director of Acute Care Services and Mobile Crisis and is a community based role.
- Bachelors Degree in Human Services or a related field preferred or high school education and life experience related to assisting homeless persons and persons with mental illness.
- Minimum of two years of experience working with homeless individuals and/or the SMI/SPMI population is highly recommended.
