- Responsibilities Complete required data entry and paperwork in accordance with program and agency policies and procedures, including but not limited to opening requirements, assessments, IMCANS, continued stay reviews, progress notes, and any other funder requirements: Screen and assess for appropriate integrated care services including substance use, mental health, and primary care.
- Provide services identified in treatment/recovery/safety plan: Especially specialized case management services including transportation, home-based, and virtual clinical services as needed to address barriers to consistent treatment attendance and engagement.
- Facilitate random client drug screens as recommended for treatment and required by the funder.
- Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness by assisting clients/families with regaining control over their own lives through self-directed recovery, including utilizing recovery language in all applicable documentation.
- Provide for referral, linkage, and advocacy to recommended Chestnut and other community resources: Consult with co-workers and DCFS personnel to coordinate effective referrals.
