- Provide case management services to participants in a program serving homeless individuals residing at the Emergency Triage Center in Worcester.
- Ensure adherence to policies and protocols for programs serving homeless individuals, including guidelines for meals, curfews, meeting attendance, client transportation to appointments, etc.
- Ensure all participants meet the HUD requirements for entry into the program and that all required documentation is included in the client file.
- Work closely with Shelter staff, Relapse Prevention Managers, and Sober Housing staff to identify new program participants.
- Provide crisis intervention (24-hour cell phone coverage).
