- The Case Management Benefits Specialist provides outpatient mental health services including assessment, treatment and care planning, resource identification, benefit coordination, service monitoring, and community-based functional support and counseling.
- Provide education and support to educate and create awareness of benefit resources such as but not limited to Aid to the Permanent and Totally Disabled (APTD), Women, Infant and Children's (WIC) Nutrition Program, Social Security, and Medicaid;
- Support clients and staff requesting NHEASY CLM access to view and/or manage their Medicaid and other DHHS benefits online;
- Work collaboratively with members of the treatment team, Intake Clinicians, Social Security Administration, Department of Health and Human Services (DHHS), clients, natural supports, and other community partners to ensure a wrap-around approach is provided for continuity of client care;
- Provide overall management of the spenddown population including but not limited to entering spenddown amounts in EHR, monitoring internal and external expenses incurred towards spenddown, timely submission of invoices to DHHS; monitoring retro-active eligibility, billing and eligibility status for spenddown cases.
