- The Case Management/Assessment Specialist will develop and implement activities supporting substance abuse prevention, intervention and rehabilitation, and perform other related work.
- Use the American Society of Addiction Medicine (ASAM) guidelines for placement and determination of stage of change.
- Provide intensive case management including transportation, client advocacy, care coordination with other care providers and linkages to other housing support services, benefits assistance, employment services, peer services, supported education and psychosocial rehabilitation services, and client-driven treatment planning.
- Refer and connect clients to a continuum of substance use disorder and mental health services, including Medication Assisted Treatment, detox facilities, and other SUD treatment options.
- Assist the Grant clients with housing readiness and referrals to permanent housing navigator/locator services and supporting them to obtain CES assessment for prioritizations of housing and eligibility for housing vouchers.
