Job Description & Requirements Specialty: Home Health Discipline: RN Start Date: 04/20/2026 Duration: 12 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel JOB DESCRIPTION ● About Facility: o Home Health Services – serving the following areas: Cleveland, Gaston, Rutherford Counties o Radius rule: Traveler's permanent address must be 50 miles outside of home unit ● Department Information: o Typical Caseload: ▪ Average patients seen/day: (visits are weighted) 6-7 units/day; 30/week ▪ Average Admissions completed/day or week: Admissions are weighted at 2.40 units – 2 admissions can be expected per day o Minimum Years of Experience required: 1 year; Home Health experience is preferred o Will you accept a first-time traveler? Yes – if they have had Home Health experience as a nurse ● Patient Types/Common Diagnoses: o 18 y/o (No Peds), wound care, wound vacs, Pleurx drains, cath (foley and SP), feeding tubes, IVs (peripheral and PICCs/Ports), Diabetes Mellitus, Heart Failure, Medication and Disease mgmt ● Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance: o RN license, BLS, driver's license, proof of vehicle insurance ● Skills required: o Homecare Homebase charting experience is required, Oasis, hospice philosophy, infection control practices, wound vacs, wound care, PICC line dressing changes, port access, IV infusions, drains, critical thinking skills, catheter insertion, drains o Ability to drive at night ● Support within the Department: o Leadership o LPN/CNA o Peers o Case Management ● Technology/Equipment: o EMR: HCHB (Homecare Homebase) ● Orientation (facility and unit): o NEO o 2 days departmental ● Scheduling: o Mon-Fri 8a-5p ▪ Weekends and Holidays dependent on needs and discussed during interview o RTO: Manager approval ● Other notes: o Scrub Color: Navy TB_RN ZRHC Talent Burst Job ID 26-06704.
