Whether you serve patients directly or support those who do, your role helps bring out the courage in others.Job SummaryProvide skilled, patient-focused home health care that supports safety, recovery, and independence at home.Job Specific DutiesConduct initial and ongoing assessments; develop, implement, and revise individualized plans of care based on patient condition and progress, in accordance with physician orders and regulatory deadlinesProvide skilled nursing services and specialized procedures; complete accurate and timely clinical documentation (including OASIS assessments) to support compliance and coding accuracyNotify physicians of patient condition changes; process orders, certifications, recertifications, and discharge summariesProvide direct patient care and on-call coverage as assigned; promptly report incidents, variances, or complaintsSupervise and train LPNs and HHAs, including required supervisory visits, retaining accountability for delegated tasksEducate patients and families on health needs, safety, infection control, emergency preparedness, and patient rightsCoordinate services with the interdisciplinary team; participate in care conferences, QAPI activities, and peer reviewMaintain current licensure, CPR certification, and required continuing education in compliance with state and federal regulationsOther Duties:Provide care and services in accordance with infection control, safety, and risk prevention protocolsAccurately document clinical interventions, care coordination, and communication in a timely mannerParticipate in interdisciplinary meetings, case reviews, and continuing education opportunitiesMaintain compliance with HIPAA and all company privacy and confidentiality standardsRepresent the organization positively and professionally in patients' homes and community settingsWork independently with minimal daily oversight and demonstrate initiative in completing responsibilitiesProvide courteous, timely, and professional customer service to patients, families, and partnersExhibit adaptability to changing responsibilities and flexibility in completing assignmentsDemonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platformsAdhere to all organizational policies, including job descriptions, mission, and the Employee HandbookCommunicate and interact professionally and respectfully with others to support team goalsPerform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the companyWork EnvironmentPrimarily works in the fieldSupervisorClinical Manager - Home HealthSupervisesNoneRisk LevelPatient care or physical work with injury riskRequirementsQualificationsCurrent RN license in the state of practiceMinimum of one year of clinical nursing experience, preferably in home health or hospiceProficient in OASIS documentation and home health regulationsStrong clinical judgment, organizational skills, and attention to detailAbility to work independently in the field with reliable transportationCurrent CPR certificationEffective communication and interpersonal skills for patient and family educationCommitment to maintaining regulatory and documentation complianceReliable transportationValid driver's licenseAuto insurance for home visitsPhysical RequirementsMust be able to stand, walk, bend, and assist with mobility or equipment as needed. Requires the ability to perform heavy lifting and to travel locally in various weather conditions.
