V-ACT Prgm Compliance Coord

NATURE OF WORK: The ACT Program Compliance Coordinators duties include completion of assessments/re-assessments for all program clients; completion of Weekly Reviews for all program clients; obtaining authorization for services for all program clients; coordinating treatment team meetings in preparation for ACTion Plan meetings for all program clients; completion of quarterly ACTion Plans for all program clients; initiates, coordinates, and facilitates ACTion Plan meetings for all program clients. The Compliance Coordinator will ensure program documentation meets all internal and external compliance standards on an on-going basis. The Compliance Coordinator will also assist the team with engagement of consumers and will assist in locating consumers that have been out of contact with the team.The Compliance Coordinator serves as an information source and interfaces with external services ensuring quality of availability, continuity, flexibility, and integration of necessary resources and services.ESSENTIAL DUTIES:Assess clients needs and evaluate progress toward current ACTion Plan goals/objectives.Completion of all Intake Assessments for incoming program clients.Completion of all Re-Assessments for all program clients.Obtains authorization for services for all program clients in a timely manner.Completion of ACTion plans and reviews current for all program clients.Initiates, coordinates and facilitates interdisciplinary treatment team meetings.Use clear communication skills with clients and other professionals.Assist in identification of necessary resources and services for program clients.Completion of all Weekly Reviews for all Program Clients.Completes and submits documentation as required by Valley, Medicaid, Waiver, and any other licensing agencies. Employee ensures that documentation (such as Progress Notes, SALs, Timesheets, Task Analysis Forms, etc.) are completed accurately and timely and will reflect services the client(s) has received as appropriate.Reviews client charts and documentation from other programs or service providers on a regular basis to ensure coordination of services in preparation for upcoming ACTion Planning meetings.Provides crisis services and intervention and/or assistance with consideration of the principles of client choice and least restrictive alternatives.Supports and helps in the training of new employees as assigned.Assists in locating program clients and engaging program clients that have been out of contact with program staff, as assigned.Assists in the transportation of program clients to and from appointments/needed resources, as assigned.Attends all mandated trainings related to profession and enhances knowledge and/or skills which include Relias Learning Trainings.Adheres to Valleys Standards of Conduct and Compliance and follows ethical guidelines of their profession.Staff spends time participating in group or individual supervision with his/her supervisor. It is expected that clinical staff will seek supervision/ consultation at both scheduled and unscheduled times. For employees whose license requires specific supervision, employee has met requirements for clinical supervision based on licensure definitions.Meets Direct Client Service Time standards of the High Intensity Services Division.Promotes appropriate community interactions by handling public interactions in a professional and competent manner. Returns telephone calls and requests for information in a timely manner, promotes a positive image of the agency, and maintains a good rapport with agency contacts.OTHER DUTIES AS ASSIGNED.
