The RN performs all steps of the nursing process including assessing patients, interpreting data, planning, implementing, and evaluating care; coordinating care with other providers; and teaching the patient and family the knowledge and skills needed to manage their care to prevent complications.Using the latest in evidence based practice, cutting edge technology and the power of a dedicated, supportive team, the RN collaborates with patients and providers to create healthcare that empowers patients to take an active role in their health. Our nurses research, learn, teach and inspire.This position will be a 4x10 shift, Monday-Friday from 8am-6:30pm with one weekday off.ESSENTIAL JOB FUNCTIONSApplies the nursing process to evaluate patient health. This may include scheduled visits, walk-in patients and patient contact through phone or other remote technology, including outreach calls when patient's chief complaints in appointment notes warrant further triage.Assists with high acuity office visits: Initial patient intake, Point of care testing, Wound care, Assisting in procedures, Blood draws, EKGs, Discharge education and follow-upCollaborates with team members to implement a plan of care appropriate to the current situation. Obtains medical records as necessaryProvides clinical oversight and training to Medical Assistants. Will fill in for all MA tasks as needed, including rooming for providers and inbox coverage.Triages patients (phone, chat, walk-in); Coordinates same-day care and continuing care with respective care team and/or services appropriately; Educate and counsel patients on treatment options based on their reported signs, medical history and symptoms; Follow up with patients who have been deferred to ER or urgent care.Collaborates with other RNs and delegates to MAs where appropriate to cover daily RN scheduleUtilizes scope of existing standing orders and applicable nursing diagnoses.Dispenses and administers medications.Manage medication refills for care team. When appropriate, submits refills in accordance with existing standing orders and completes prior authorizationsImplements appropriate patient education programs to meet the individual health care needs.Assess patient complaints that might be escalated from Care Navigators, Care Coordinators. and Medical AssistantsPrioritize live phone calls from pharmacies, laboratories and specialists offices, etcWork closely within care team to close patient care gaps and meet established quality metrics for patient careDocuments all patient interactions following clinic guidelines.Continually evaluates overall patient care flow and intervenes to maintain optimum time lines.Anticipates potential problems/obstacles in daily operations and intervenes effectively.Continually works to achieve the highest level of patient satisfaction.Maintains the strictest confidentiality.The essential job duties outlined above will be performed in the following areas of service, as assigned, and may include the additional duties outlined below:Travel RN - Coordinate delivery of services specific to travel.Dermatology RN - Coordinate delivery of dermatology services.Pathways Program RN - Provide education and treat to target services for program participants enrolled in care pathways. Accountable for health outcomes of a defined population. Works closely with the Population Health RN to identify eligible patients, to monitor defined health metrics and provide outreach to improve patient outcomes.Population Health RN - improving the health status and care for individuals with chronic conditions; potentially complex medical, mental health, and psychosocial issuesResearch RN - Functions as a registered nurse in support of research projects within the medical group and in collaboration with outside partners. In doing so they assess and treat research subjects per project guidelines and applicable standardized procedures.NON-ESSENTIAL JOB FUNCTIONSMakes suggestions for improving operations and workflows and implementing efforts to improve the quality of care in a cost-effective manner.Engages in other duties as assignedQUALIFICATIONS AND REQUIREMENTSCurrent California RN license.Registered Nurse degree from accredited school.3 years of clinical experience.Current Basic Life Support (BLS) for health care provider.Compliance with AC Wellness Occupational Health Policy, including required immunizations.Demonstrates comfort and enthusiasm for new methods of care delivery using technology.Models excellent communication and problem solving skills with all staff, patients, providers and others by using a positive team approach.Promotes excellent teamwork, working cross functionally across roles and clinicsServes as a role model for professional behavior in the delivery of patient care.
