Job DetailsFusion Medical Staffing is seeking a skilled Home Health RN for a 13-week travel assignment in Charlottesville, Virginia. As a member of our team, you'll have the opportunity to make a positive impact on the lives of patients while enjoying competitive pay, comprehensive benefits, and the support of a dedicated clinical team.Required Qualifications:One year of recent experience as a Registered Nurse, including a minimum of six months in a Home Health setting as an RNProficient in OASIS documentationValid RN license in compliance with state regulationsBLS certification (AHA/ARC)Preferred Qualifications:* Home Health Nursing Certification (RN-BC)* Other certifications and licenses may be required for this positionSummary:A Home Health Registered Nurse (RN) delivers direct nursing care to patients in their homes, ensuring comfort, stability, and effective management of their medical conditions. Home Health RNs assess patient conditions, develop care plans, administer treatments, and educate patients and caregivers on disease management.Essential Work Functions:Assess patient conditions and develop individualized care plans based on medical needs and physician directivesAdminister medications, treatments, and wound care while monitoring patient responsesProvide patient and caregiver education on medication management, disease progression, and self-care techniquesMonitor for changes in patient status, reporting concerns and adjusting care plans as neededCoordinate with physicians, therapists, and other healthcare professionals to ensure seamless care transitionsDocument patient care, interventions, and progress accurately and in compliance with regulatory standardsEnsure compliance with infection control, safety, and home care best practices to protect patients and caregiversSupport families and caregivers, offering guidance and emotional support throughout the care processMaintain communication with case managers and interdisciplinary teams to provide coordinated patient careComplete charting and documentation requirements while meeting visit and productivity goalsAdhere to infection control, safety, and healthcare regulationsPerform other duties as assigned within the scope of practiceRequired Essential Skills:Critical thinking, service excellence, and good interpersonal communication skillsThe ability to read, write, and communicate in the English languageAbility to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skillsPhysical Abilities - Must be able to remain in a stationary position, move about, move equipment (50-100lbs), push, pull, and bendInterpersonal Skills - Must be able to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalismTechnical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipmentMental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure. Must be able to work under close supervision occasionally, as well as working without assistance from other personnel.
