Description Summary: Secures background information from patients and their families for social evaluation of environment and family relationships. Discusses information obtained in patient and family interviews with physicians, nursing staff, and other hospital and community personnel. Assists patients in finding resources they will need in their homes following discharge. Acts in a liaison role between hospital and family when patient is transferred to other hospital facilities Responsibilities: Utilize assessment data to ascertain psychosocial risk factors and develop individualized treatment plans for each patient Responsible for implementing clinical interventions that promote, maintain or restore mental health, effect rehabilitation and facilitate developmental progression Requirements: Education/Skills Master's degree in Social Work (MSW), Counseling, Psychology, or a related behavioral health field required Experience Requires a minimum of three (3) years of experience providing behavioral health, social services, or counseling support in a healthcare, hospital, or community-based setting, including experience conducting patient and family assessments and coordinating care or resources. Requires demonstrated experience collaborating with interdisciplinary care teams, including physicians, nursing staff, and community agencies, to support discharge planning, care transitions, and continuity of care. Licenses, Registrations, or Certifications Current, unrestricted licensure in the State of Texas, Louisiana, or New Mexico is required, or the ability to obtain licensure in the applicable state prior to start date. Acceptable licenses may include Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Mental Health Counselor (LMHC), or equivalent state-recognized behavioral health licensure, consistent with scope of practice and role responsibilities.
