The RN - Home Health works in collaboration with the interdisciplinary team, physician, and family to develop a comprehensive plan of care to meet the patient's needs. As a PRN (as-needed) position, the RN - Home Health will be called upon to work on an as-needed basis, based on the needs of the organization.Responsibilities:Ensure patient safety and provide high-quality nursing careAssess the patient's condition and implement the care plan in collaboration with the interdisciplinary teamProvide wound care, medication management, IV therapy, and other skilled nursing servicesEducate the patient and caregiver on disease management, prevention of complications, and self-careCollaborate with physicians and other healthcare professionals to develop and implement the plan of careComplete documentation of patient care in an accurate and timely mannerParticipate in quality assurance and performance improvement activitiesAttend and participate in interdisciplinary team meetings, as neededQualifications:Active Texas RN licenseAt least 1 year of nursing experience, with Home Health experience preferredAbility to work independently, with minimal supervisionStrong clinical and critical thinking skillsExcellent communication and interpersonal skillsReliable transportationAbility to work on an as-needed basisPhysical Demands:The RN - Home Health must be able to perform physical activities that require considerable use of arms, legs, and moving the whole body, such as lifting, balancing, walking, stooping, and handling of materials. The RN - Home Health must also be able to navigate different home environments and adapt to various indoor and outdoor conditions.This is a PRN (as-needed) position.
