Family Practice - Geriatrics Physician

Job Title: Family / Internal / Geriatric Medicine PhysicianLocation: Minneapolis, MNShift: Full-time & Part-time position Summary:We are looking for a Physician to serve patients in Long-Term Care (LTC) and Transitional Care Units (TCUs). Responsibilities:Conduct comprehensive patient assessments and ongoing medical evaluations.Create and manage personalized treatment plans based on patient health needs.Diagnose and treat acute and chronic conditions, providing timely medical interventions.Prescribe medications, order diagnostic tests, and modify treatments as needed.Work closely with nurses, therapists, and social workers to coordinate comprehensive care.Requirements:MD or DO degree from an accredited medical school.Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics preferred.Valid medical license to practice in Minnesota.Previous experience in long-term care, transitional care, or geriatrics is a plus.Excellent problem-solving, communication, and teamwork skills.For more info, please reach me at / .
