Sr Occupational Therapist

Job Summary and ResponsibilitiesAs our Occupational Therapist, you will provide essential in-home occupational therapy services under physician's orders. Your role involves evaluating home safety, assessing functional needs, and implementing tailored treatment to promote independence.Every day you will evaluate home safety, assess functional needs of physically disabled patients, then develop, plan, and implement individualized occupational therapy treatment procedures.To be successful, you will demonstrate strong assessment, treatment planning, and implementation skills for in-home care. Excellent communication for patient education and a commitment to promoting independence and safety are essential.Appropriately assesses patients of varying diagnoses & disorders requiring Occupational Therapy according to established guidelines. Age range treated includes pediatric, adolescent, adult, & geriatric. Staff will be assigned only age ranges for which they meet education, experience & training.Patient's age 0-21 with CCS insurance will either be seen by CCS paneled therapists or have oversight by a CCS paneled therapist.Establishes functional goals in collaboration with the interdisciplinary team based on the patient's rehabilitation potential.Develops, implements & modifies treatment plan in collaboration with interdisciplinary team to achieve established goals. Utilizes evidence based practice as available. Incorporates patient/family/Care Partner needs, interests & goals in the development & execution on the treatment plan.Provides patient/family/Care Partner/caregiver education, initiating as soon as feasible after admission. Works to ensure transfer of knowledge using a variety of techniques (i.e. teach back, demonstration, video, written handouts).Establishes/follows patient precautions. Observes patient's response to treatment & reports unusual occurrences. Follows hand off guidelines.Job RequirementsBachelor's or higher degree Occupational Therapy and must have a minimum of 10 yrs. as a practicing OT, for part-time/full-time staff, including demonstrated teaching responsibilities & evidence of continuing education, upon hire andCasual per diem staff minimum of 2 yrs. experience andPart-time/full-time staff must demonstrate advanced clinical expertise in a specialized area andNew applicants must present documentation of continuing education or experience. andUpon annual appraisal, must demonstrate participation in external continuing education or experience. andExperienced therapists with 1 year equivalent of pediatric experience will apply/obtain CCS paneling within 6 months of hire.Occupational Therapist: CA, upon hire andBasic Life Support - CPR required.Fire Safety, within 90 Days required.Where You'll WorkFounded in 1955, Dignity Health - Northridge Hospital Medical Center is a 394-bed, acute care, nonprofit community hospital located in Northridge, California.
