The Admissions Coordinator for Home Health Care plays a key role in supporting that mission by serving as the primary liaison between Meritus Medical Center and partnering Home Health agencies. This role ensures patients receive timely, appropriate, and well‑coordinated post‑acute services that support recovery, safety, and continuity of care.Under the supervision of the Clinical Manager, the Admissions Coordinator identifies potential referrals, assesses eligibility for home health services, educates patients and families, and collaborates closely with physicians and the care management team.
