Summary Under minimal supervision, provides nursing care for a group of patients assigned to the nurse based on matching the patients' needs with the nurse' competencies. Completes established competencies for the position within designated introductory period. Other related duties as assigned. ESSENTIAL / PRIMARY DUTIES (The essential job functions or primary responsibilities that must be performed unaided or with the assistance of an accommodation - all job functions should begin with an action verb) General Functions:Assesses human responses and adaptation to actual or potential health problems of the patient, family and/or significant otherInstitutes and initiates a plan for patient care based on the initial assessment; implements and continuously evaluates interventions; reassesses patient needs on an on-going basis.Recognizes changing patient condition and utilizes unit resources, chain of command and Rapid Response Team (RRT) in a timely and appropriate mannerDocuments all plans and care providedIdentifies need for education of patient, family and/or significant other and provides education through individualized instruction, the use of written materials and/or other available resourcesAssigns nursing care to licensed staff, delegates nursing care to unlicensed staff and coordinates the flow of workCalculates and administers medicationsCoordinates and collaborates with multidisciplinary team members in the planning and management of patient careParticipates in discharge planning; provides instruction to patient/familyMaintains and promotes professional standards and development by participating in inservices, committees, staff meetingsServes as a role model for compassionate care and service excellence and commits to Studer Principles and must havesFacilitates patient movement within the department and through the facility, proactively managing patient admissions, discharges and transfers.Manages patient hand-offs at shift change and between department and facilities; utilizes documentation tools for hand-offs as requiredProvides every patient the defined standard of care (ie; core measures, chest pain protocol)Utilizes resources efficiently and effectivelyMaintains safe environmentAddresses patient population-specific needs (clinical)Participates in Performance Improvement Activities Join our dedicated healthcare team where compassion meets innovation! MINIMUM EDUCATION: Graduate of accredited school of nursing. PREFERRED EDUCATION : Bachelors Degree MINIMUM EXPERIENCE : None for Med/Surg and Behavioral Health 1 year in acute/chronic dialysis for Dialysis PREFERRED EXPERIENCE : 2 years in acute care for Med/Surg 2 years in psychiatric facility for Behavioral Health 2 years acute/chronic dialysis for Dialysis REQUIRED CERTIFICATIONS/LICENSURE : Possession of current Texas State License for Registered Nurse. For RN in Behavioral Health: Non-Violent Crisis Intervention must be obtained within 60 days of hire or transfer into department PREFERRED CERTIFICATIONS/LICENSURE: REQUIRED CCOURSES/COMPLETIONS (e.g., CPR): Must have active healthcare provider Basic Life Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire.
