Health Center is seeking a Family Practice physician to work at our clinics.-The Center is Federally Qualified Health Centers (FQHC) and provides a network of Family Practitioners, Internists, Psychiatrists, Dentists, and Social Workers.--Empower patients to take an active role in health maintenance and promote wellness behaviors--Provide appropriate patient follow-up and communicate effectively with patients--Refer patients to specialty care as necessary--Provide clinical supervision to mid-level providers and nursing staff--Participate in quality assurance activities--Participate in State and National disease collaborative efforts--Community Health Center sites are certified Level III Medical Homes, with a highly developed and supportive panel management and case management system in place.--Robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites.-EMR: Medent.--Market leading compensation ? commensurate with experience.--Generous bonus opportunity.--Up to $10,000 in relocation expenses.--Medical, dental, and vision insurance.--Retirement plan ? 403(b) with employer match.--Paid malpractice insurance.--Generous leave time: starting at 4 weeks plus holidays with an increase after 18 months.--Work-life balance with 4-day work weeks.--On-Call 3 weeks per year on average.--CME leave (1 week/year) and CME allowance ($6,000/year)--Opportunities to apply for school loan repayment, up to $20,000 a year.
