We Welcome You.We are open to RNs without prior home health experience and provide comprehensive training, mentorship, and ongoing clinical support to help you transition confidently into the field.Key Responsibilities• Perform initial and ongoing patient assessments to determine skilled nursing needs • Complete Start of Care (SOC), Resumption of Care (ROC), Recertifications, and Discharges accurately and timely • Provide skilled nursing services including wound care, IV therapy, injections, catheter care, and medication management • Educate patients and caregivers on disease processes, care plans, and safety protocols • Coordinate care with physicians, therapists, social workers, and interdisciplinary team members • Serve as Case Manager for assigned patients (as applicable) • Document all care accurately in OASIS-E using EMR (WellSky) • Supervise and document care provided by LVNs and Home Health Aides (HHAs) • Participate in case conferences, team meetings, and quality improvement activities • Ensure compliance with Medicare Conditions of Participation and California Title 22 regulationsQualifications• Active and unrestricted California RN license • Graduate of an accredited nursing program • Minimum 1 year of RN experience (home health experience not required) • New to home health?
