Family Practice - Geriatrics Physician

We are seeking Physicians and Nurse Practitioners to provide on-site primary care to residents in a skilled nursing facility serving veterans. The provider will deliver high-quality, integrated care, coordinate with nursing staff and the Medical Director, and maintain comprehensive documentation in the electronic medical record.Job Type: Contract / On-Site Provider (Physician or NP)Specialty: Family Medicine, Internal Medicine, or Geriatrics Responsibilities Provide primary care services on-site to residents, including admissions, follow-ups, care plan reviews, and discharge planningDocument all patient encounters in the electronic medical record (EMR)Provide after-hours, weekend, and holiday phone/on-call coverageRespond to staff and care team inquiries promptlyCoordinate quality-of-care efforts with the Medical DirectorParticipate in policy and program development to maintain high-quality careMaintain a 24/7 coverage plan, including backup providersEnsure compliance with federal, state, and regulatory standards (CMS, Joint Commission, VA regulations)
