Analyzes data collected.+ Plans and prioritizes individualized care based on the assessment, utilizing nursing diagnosis, including teaching and discharge needs of the patient and family, and sets outcome goals in coordination with other disciplines.+ Ensures that the patient/family participates in the plan of care as able, and that plan of care addresses age-specific and cultural needs.+ Implements appropriate standards based on assessment.+ Prioritizes and organizes care within the allotted time frames.+ Coordinates discharge steps or ensures appropriate referral to address timely, appropriate discharge needs.**Job Requirements**Required+ Two years acute care experience and must include at least one year of Telemetry experience.+ California RN license+ Basic Life Support - CPR+ Advanced Cardiac Life Support**Where You'll Work**Founded in 1958, Dignity Health - Sierra Nevada Memorial Hospital is a 104-bed, acute care, nonprofit hospital located in Grass Valley, California.
