Patient Care Manager, Registered Nurse

Home Health Patient Care ManagerAs the Home Health Patient Care Manager, you are responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.Primary Responsibilities:Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care teamReceives referrals and ensures appropriate clinician and/or therapist(s) assignments for timely patient evaluation by signing off after authorization and plotting start of care (SOC) visitsCoordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvalsEnsures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance to physician ordersManages and documents phone calls from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to cliniciansReviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriateRequired Qualifications:Current RN licensure in state of practiceCurrent CPR certification or ability to complete within 90 days of hireCurrent Driver's License, vehicle insurance, and access to a dependable vehicle or public transportationAbility to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the clientPreferred Qualifications:Home Care experiencePay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $72,800 to $130,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
