Linkage to Care Specialist, Gilead FOCUS Grant, FT, 9A-5:30P

This position plays a key role in ensuring that patients who test positive for HIV, HCV or Syphilis are able to access care and maintain that care into the future. The Linkage to Care Specialist will collaborate with resources from the Department of Health and the community to reduce barriers to patients receiving care. Estimated salary range for this position is $46176.03 - $60028.84 / year depending on experience. Degrees:* Bachelors.Additional Qualifications:Knowledge of HIV/AIDS/HCV prevention, outreach and health promotion.Experience working with people living with HIV/AIDS.Knowledge of specialized services for people with HIV/AIDS/HCV.Excellent written and verbal skills.Computer skills using Microsoft software applications (Word, Excel, Outlook, Powerpoint) and other computer systems as needed.Minimum Required Experience: 2 Years
