Oregon Licensed Case Manager

A company is looking for a Mid Case Manager. Key Responsibilities Conduct outreach, engagement, and follow-up with referred members Develop, update, and monitor Person Centered Service Plans (PCSPs) Coordinate across medical, behavioral health, and social service systems to reduce barriers to care Required Qualifications Bachelor's degree in Social Work, Public Health, Human Services, or a related field 2+ years of experience in Medicaid funded case management or care coordination Experience working with high need populations Ability to build partnerships with healthcare providers and community organizations Experience in providing technical assistance and best practice implementation support
