Job DescriptionUVM Health – Porter Medical Center Helen Porter Rehabilitation and Nursing Center in Middlebury, VT.The University of Vermont Health is seeking an outstanding full-time clinician to join our Geriatrics Division within our Department of Medicine. The successful candidate will:Responsibilities: Provide high quality care to sub-acute rehab patients and long-term care patients at Helen Porter.Participating in quality initiatives, including efforts to improve patient safety, and quality of care.Medical directorship with 4 hours of protected time weekly for administrative responsibilities.The individual will participate in shared call coverage including evenings and weekends.Interprofessional collaboration across the healthcare system to ensure coordination of patient centered care and communication.Prefer Certified Medical Director certification or enrollment within 1 year from hire to provide effective management and delivery of medical care in long-term care settings in compliance with federal and state regulations while supervising and coordinating providers to ensure adherence to the medical regulations and standards.
