We strive to empower individuals and their families by prioritizing their unique needs and fostering a holistic approach to care that treats the whole person, not just the condition.Role Description This is a flexible, field-based PRN / 1099 Contract role for a Physical Therapist specializing in Home Health, serving patients throughout San Diego County. The Physical Therapist will bridge the gap between inpatient and community-based care by evaluating patients, designing personalized Plans of Care, and providing hands-on therapy to promote mobility, relieve pain, and manage physical disabilities.You will work with diverse patient populations—including geriatric, orthopedic, and neurological cases—to ensure seamless recovery and optimal outcomes. This role involves close collaboration with patients, their families, and a multidisciplinary team of physicians and nurses. High-quality documentation and progress tracking are integral to this role to ensure the highest standard of rehabilitation.Key Responsibilities:Conduct initial evaluations and develop individualized Plans of Care.Assess muscle strength, mobility, gait, ROM, and home environment safety.Deliver skilled PT interventions including therapeutic exercise and functional rehabilitation.Educate patients and caregivers on home exercise programs and safe movement techniques.Supervise and provide clinical guidance to PTAs as applicable.Maintain accurate, timely documentation per agency and California state standards.Qualifications Education: Doctoral degree in Physical Therapy (DPT) from an accredited program.Licensure: Current Physical Therapist license in the state of California in good standing.Clinical Skills: Proficiency in Physical Therapy, Rehabilitation, and managing complex mobility challenges.Soft Skills: Exceptional interpersonal and communication skills to build strong relationships with patients and families.Compliance: Current CPR certification, TB test/Chest X-Ray, and Physical Exam (or ability to obtain prior to start date).Experience: 6+ months of home health experience is a plus; however, strong acute care or outpatient backgrounds will be considered.Mobility: Valid California driver's license, reliable vehicle, and proof of auto insurance for home visits.
