Here, hospice care is personal, and our Hospice Liaison professionals are essential ambassadors of that promise.A Day in the Life of a Hospice LiaisonAs a Hospice Liaison primarily for the DeKalb/Sycamore area but will also provide service in the Rockford and surrounding areas, your day blends purpose with strategy. You will meet with physicians, discharge planners, and community partners, using your hospice knowledge and RN perspective to educate them on services and eligibility. You’ll collaborate closely with leadership on marketing initiatives, adapt outreach strategies, and identify new opportunities to serve patients sooner. As an RN, your clinical credibility strengthens every conversation. Hospice Liaison Responsibilities: Execute daily sales and marketing strategies to referral sourcesServe as a knowledgeable liaison between providers and hospice servicesEducate healthcare professionals and the community on hospice careBuild and sustain referral relationships using RN clinical expertiseDevelop referral growth plans with the Marketing & Community Relations DirectorStay current on hospice regulations and market trendsRepresent Northern Illinois Hospice with professionalism and heartRequirements:Associate degree required (ADN); BSN preferredProfessional Registered Nurse License in the state of IllinoisHospice or palliative care experience preferred; marketing experience preferredThree (3) years nursing experience in a hospital or facility setting. DeKalb / Sycamore residency preferredStrong communication, presentation, and relationship-building skillsKnowledge of hospice regulations and community healthcare networks Valid drivers’ license, current auto insurance and access to a reliable automobile.Schedule:Full-time, Monday – Friday (daytime hours)Salary DescriptionWe support our employees’ wellbeing and regularly benchmark with other companies in our industry to offer an extensive Total Reward package for this position. The expected base salary range for this role is $79,000 – $114,000. However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data.
