- A Brief Overview The Wound Care Nurse will function in the role of a resource to staff to facilitate and provide care to patients pressure injuries, ostomies, vascular ulcers, and other wounds and will participate in staff education and performance improvement initiatives related to wound and ostomy care.
- Prioritize wound care consultations in collaboration with team members.
- Document wound care assessment data, develop/revise individualized plan of care, referrals and 4.
- Collaborate with an inter-professional team, including but limited to; physicians, nurses, nutrition, physical therapy and select wound care team members, to ensure smooth day-to-day operations within respective team, ensure high quality care, proper hand-off communication and effective and efficient resource utilization.
