As a Structural Heart APP and Program Coordinator, the PA/NP will work in an administrative and clinical capacity with collaborating physicians, clinical staff, patients and their families, insurance companies, and vendors, in the planning, evaluation, and coordination of care for our Structural Heart patients across the entire continuum of care, including pre-procedural / post-procedural care in the outpatient and inpatient clinical settings.. Serves as expert clinician resource for the Structural Heart Program at MVHS. Delivers skilled care and experience through the development, implementation, education, collaboration, and evaluation of optimal critical care pathways for the Structural Heart patient population.. Identify needed co-morbid conditions and obtain testing to satisfy risk analysis and scoring in keeping with the Society of Thoracic Surgeons (STS) organization calculator.. Assess and implement medications, policies, and documentation to satisfy STS, NYS DOH quality measures/metrics.. Current NYS Nurse Practitioner licenses OR Current NYS license as a Physician Assistant with prescription privileges.
