- Job Details Description Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions.
- SUMMARY:The LEAD Screening and Intake Coordinator works as a member of a multi-disciplinary team providing screening, outreach, and service brokerage to participants referred to the LEAD program.
- The team coordinates with community resources to broker services provide access to address client needs, including substance abuse treatment, medication management, physical health treatment, mental health treatment, benefits and entitlements, appropriate and affordable housing, employment and educational opportunities, and supports for basic needs.
- Develop and maintain positive, collaborative relationships with all LEAD partners (such as Law Enforcement, Human Services, District Attorney’s office) and other service providers in order to best serve LEAD participants and serve as direct liaison regarding specific referrals for LEAD engagement and participation in this service.
- The Intake coordinator will provide bridge/transitional CM supports until the regular assigned CM has capacity to engage the participant in longer term case management servicesManage, coordinate and facilitate the assignment/hand off of new referrals to a case manager, peer or housing.
