- The Rehabilitation Specialist will provide assistance to clients in gaining skills that are based on their individualized goals.
- These skills include, but are not limited to, medication management, developing life skills (i.e. cooking, money management, cleaning, parenting, and socializing); pursuing recreational and leisure activities, housing, transportation, legal, medical and financial assistance, and securing appropriate entitlements.
- The Rehabilitation Specialist will participate in weekly team meetings to coordinate client care and assist in developing person centered care plans.
- The Rehabilitation Specialist is also required to meet organization stands in timely and accurate documentation, which focuses on the progress and treatment of clients.
- The Rehabilitation Specialist will work with clients both in the community and in the office, but is primarily a community based position.
