- DUTIESPromotes a recovery-based approach to care that emphasizes hope, respect, self-direction, and empowerment.
- As applicable, works directly with members on a peer-driven, self-developed recovery plan to be shared with their Behavioral Health Medical Practitioner (BHMP) that includes goals of the member / family, community resources, family/natural supports, and crisis prevention plan.
- Relates own recovery process or family experience in a positive and supportive manner through coaching, role modeling and mentoring to assist clients and/or their families in developing skills to achieve their goals (if certified to provide peer or family support services).
- Assists in assessment of hospitalized/crisis clients for purpose of discharge planningPerforms welfare checks in the client’s home to assess for health and safety risks and reports clinical information to the Recovery Coach/Supervisor Attends required training.
- Coordinate all care and services required to enhance and facilitate the optimal physical and behavioral health wellness of persons served within a trauma-informed care framework.
