A Brief Overview
The Patient Care Coordinator functions as a member of the patient care team. The position performs clinical tasks and procedures to support the delivery of care and to promote efficient use of the physician's time as well as performs all clerical duties required to complete patient Check-in/Check-out. The Patient Care Coordinator is responsible for assisting patients obtain services in a timely manner and assuring efficient utilization of the physician's appointment schedule and in the coordination of patient care. This position administers and coordinates referrals and service authorization process, rooming duties and performs other clinic front desk activities. Patient Care Coordinator has five levels that are distinguished from each other primarily by: 1. Entry: The complexity of assigned duties and responsibilities. 2. Proficient: Knowledge level of required clinical care skills. 3. Full Proficiency: Required level of business communications skills. (Department must have business need to progress) 4. Expert: Independence of action (degree of supervision received). (Department must have business need to progress) 5. Lead: Independence of action (degree of supervision received). Specialized tech knowledge (i.e., procedural area,) OR Care Coordination • Primary Care Panel Management (PC=1500 panel size w/Population Health outreach) • Specialty Panel Management (defined population with measurable outreach & protocol-based f/u of care gap closure) • Appointment/procedure coordination for specific population of patients who require coordination across system (at least 25% of time) • Specialized data collection for care plan management of complex patients (documentation support, pop health, etc.) 3. Clinic Support • Preceptor/Orientation • Annual or New Competency Assessment • Super user • MA assignment support • Pend orders (route to Provider to finalize) • Real-time documentation "Level I"

What you will do
<ul>
<li>Patient Care</li>
<li>Prepares the necessary information (i.e. lab and procedural results), equipment and supplies for exam.</li>
<li>Assists physicians with procedure set-ups, patient procedures and examinations.</li>
<li>Performs patient care, including vital signs, weights, rooming, assisting with procedures, medication administration (vaccinations) and injections and other necessary tests.</li>
<li>Administers medication following approved procedures and only after checking medication and dose with a licensed person.</li>
<li>Processes requests for prescription refills according to established practice.</li>
<li>As required, performs waived testing (urine dipstick, urine pregnancy test, blood glucose testing), perform EKGs, administer immunizations.</li>
<li>Collects and sends specimens to appropriate labs for testing per protocol.</li>
<li>Assists the physician with maintaining preventive health measures for a panel of patients.</li>
<li>Promptly and accurately reports and refers patient's concerns and symptoms, reactions, changes (appropriate to age) to licensed clinic personnel.</li>
<li>Scheduling</li>
<li>Schedules and edits and maintains patient physician appointments for new and returning patients, using the scheduling system.</li>
<li>Schedule radiology appointments.</li>
<li>Customer Service</li>
<li>Maintains clean and orderly exam rooms, hallways and work areas.</li>
<li>Effectively responds to inquiries from patients, their relatives and/or referring physicians on services and provides accurate information in a timely fashion.</li>
<li>Provides information, referrals and customer service</li>
<li>Acts as a patient advocate, coordinator and liaison for Stanford Health Care in dealing with the community.</li>
<li>Acts as non-clinical liaison/concierge/navigator for the referred patients in any way the patient needs before, during or after an encounter with a Stanford medical facility.</li>
<li>Clerical Duties</li>
<li>Informs patient's authorization and collects payment for new and returning patients, using the scheduling system.</li>
<li>Collects patient demographic and insurance data and enters into systems as well as assigns medical record numbers</li>
<li>Assures appropriate verifications and authorizations are complete before patient sees provider</li>
<li>Assists with all messaging from the patient to the physician in a timely manner.</li>

Education Qualifications
<ul>
<li>High School Diploma or GED Required</li>
<li>Medical Assistant Certificate from an approved school /institution or equivalent documented training Required</li>
<li>Bachelor's Degree Preferred</li>
</ul>
Experience Qualifications
<ul>
<li>3+ years to 5 years of directly related experience Required</li>
<li>+6 months as PCCII at SHC Required</li>
</ul>
Required Knowledge, Skills and Abilities
<ul>
<li>All levels require possession of knowledge, skills and abilities (job competencies) sufficient to function effectively at the level to which assigned.</li>
<li>Knowledge of and ability to use and apply medical terminology in performing his/her duties.</li>
<li>Knowledge of coding and billing regulations.</li>

<li>Demonstrated familiarity, knowledge and understanding of home health, practice care coordination, clinical care skills.</li>
<li>Ability to demonstrate business communication skills including speak and write effectively at a level appropriate for the job (including legible penmanship/handwriting, knowledge of grammar and syntax and ability to write clear and concise clinical reports)</li>
<li>Ability to learn and grasp and update clinical knowledge on a regular basis (through HealthStream classes, continuing education and other work related courses and training) to adapt to an ever changing healthcare environment.</li>
<li>Ability to learn and understand population health and disease population concepts and practices; inventory methods and practices; motivation learning; customer relationships service concepts and practices.</li>

Base Pay Scale: Generally starting at $33.14 - $37.30 per hour
The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
