- The Supportive Care Nurse Practitioner (APNP) is a member of the medical services team and the palliative care consultative team.
- This position collaborates with patient’s primary care providers in the assessment, planning, implementation and evaluation of community-based palliative care interventions.
- This position is responsible for interdisciplinary collaborative practice, ongoing quality assessment and performance improvement, performance management, financial management, and ensuring exceptional care delivery consistent with hospice/palliative care philosophy and best practice.
- This position requires in-home visits with our patients along with telemedicine visits as needed.
- Current WI RN license, Advanced Practice Nurse Prescriber (APNP) licensure, and national AANP or ANCC NP certification.
