- The Manager of Population Health Operations leads a team of care coordination staff to ensure optimal care for patients, including those with complex medical conditions, patients being discharged from the hospital and patients needing preventive screenings.
- The manager collaborates with administrators, medical staff, and other disciplines to ensure alignment with evidence-based population health management practices (NCQA PCMH and Health Equity Accreditations).
- Manages key population health management programs such as Complex Care Management (CCM), Transitional Care Management (TCM), and Quality Improvement for Value-based Care Contracts including, but not limited to workflow optimization, program reporting, and outcomes measurement.
- Understands and utilizes healthcare knowledge and experience to perform various independent analyses to support the design, implementation, and evaluation of population health management programs.
- Effectively represent the department in meetings, conferences and other activities related to population health care management operations and related domains as assigned.
