Job Description: LVN/LPN Under the direction of the RN Supervisor, the LPN Wing Supervisor carries out the medical plan of care and oversees the Nursing Assistants' care of residents and High School Students to insure that resident care is carried out in accordance with good nursing practice and applicable State and Federal regulations. Client's ideal candidate would be someone who has a passion to work with and interact with residents daily. GENERAL DUTIES: <ul> <li>Physician's Computer Company: EHR/EMR</li> <li>Administers and records medication, treatments and monitoring e.g. glucose checks, as ordered and reports and documents resident response</li> <li>Accurately transcribes physician's orders</li> <li>Completes monthly summaries as scheduled</li> <li>Reviews the care plan with NAs on monthly summary day and updates the care plan to reflect the resident's current condition.</li> <li>Notifies the RN Supervisor immediately of a significant change in resident's condition</li> <li>Reports and records changes in resident condition to the RN Supervisor</li> <li>Communicates resident transfers to immediate family and via email to department heads</li> <li>Completes transfer form for all transfers</li> <li>Updates care plans with clear, concise, time limited goals to reflect resident condition and directs team to implement approaches</li> <li>Assists physicians with rounds, examinations and treatments as needed</li> <li>Collects laboratory specimens e.g.: urine, cultures, stool, etc. and arranges for delivery to Hospital with accurate and complete requisitions. Documents in PCC when laboratory work is done</li> <li>On resident death, supervises and assists with post mortem procedures. Assists with collection of resident's clothing and belongings. Notifies all department heads of death via email, makes phone calls as directed by RN Supervisor</li> <li>Encourages resident participation in scheduled activities</li> <li>Reports to team changes in resident condition, care plan changes and any special assignments to be carried out</li> <li>Assists RN Supervisor with annual evaluations of nursing assistants</li> <li>Along with RN Supervisor, writes commendations and counseling records as needed</li> <li>Accurately completes incident reports or assists team members in writing report</li> <li>Documents weights and vital signs and follows up as needed, including reweighing residents if there is a three pound or more discrepancy from the last weight</li> <li>Cleans and stocks treatment trays daily</li> <li>Prepares daily report for the wing</li> <li>Counts narcotics with another nurse at beginning and end of shift</li> <li>Maintains an accurate inventory of resident belongings - notes new items brought in and items removed from facility</li> <li>Maintains work area in clean, orderly condition. Performs housekeeping assignments as scheduled</li> <li>Submits maintenance requests as needed</li> <li>Maintains a clean, safe and healthy environment and reports potential safety hazards</li> <li>Supervises and directs resident care by nursing assistants to insure the prescribed plan of care is carried out</li> <li>Performs additional duties as directed by supervisor</li> <li>Attends care plan conference for residents on assigned wing if requested</li> <li>Assists in orientation of new nursing staff</li> <li>Adjusts staffing assignments as needed to insure good resident care</li> <li>Conducts team conferences as needed</li> <li>Assures that staff is familiar with changes in policies and procedures</li> <li>Updates the daily census as needed</li> <li>Monitors NA documentation for completeness and compliance with regulations</li> <li>Collects data for quality assurance studies as directed</li> <li>Monitors nurse aide job performance for compliance with infection control practices</li> <li>Provides direct care for residents as needed, within restrictions</li> <li>Seeks clarification or guidance to perform job responsibilities when in doubt</li> <li>Documents drug disposal on resident charts in PCC.</li> <li>Attends in-services, department and staff meetings, committee meetings as required</li> <li>Assist CNA's on the hall as needed</li> </ul> PHYSICAL DEMANDS: <ul> <li>Good physical health and mental alertness to be capable of performing job functions and complete work assignments in a timely manner</li> <li>Ability to pass employment physicals and as required</li> <li>Knowledge of principles of good body mechanics</li> <li>Have physician permission to perform duties outlined in the transitional duty job description</li> <li>Must be free of communicable disease</li> </ul> Experience: <ul> <li>Nursing Home experience a plus</li> <li>COVID Vaccine - Preferred but not required</li> <li>Must possess knowledge of: medical asepsis, body mechanics, personal care of residents, correct use and care of equipment, observation techniques, reporting and recording, thorough knowledge of nursing theory and it's application</li> <li>Must be willing to possess knowledge of and comply with regulations, policies, procedures, contracts and philosophy of the institution</li> <li>Must maintain a courteous and professional manner toward residents, visitors and co-workers and a dedication to meet the needs of the elderly</li> <li>Must have awareness of cost containment, utilize time fully, and properly care for equipment</li> <li>Must possess ability to accept and follow written and verbal directives</li> <li>Must utilize good communication and observation skills</li> </ul> Certification Requirements: BLS State License Requirements: Pennsylvania Additional License Requirements: Must have Current PA license or have applied for PA license at the time of interview!
