- The primary purpose of your job position as the RN Case Manager is to perform assigned duties in an efficient manner, in accordance with the established procedures, and as directed by your supervisor, to assure that a successful, viable department is maintained at all times, hence, contributing to creation of a positive atmosphere when dealing with the patients, physicians, payers, and all hospital personnel.
- The position of Social Work Case Manager I is a qualified professional, who provides assessment, counseling, education and information resource referrals and the coordination of services which meet the psychosocial and health related needs of child/adult/geriatric having medical, mental health disorders, families, and or others.
- Registered Nurse or Master’s Degree (MSW) from a school of Social Work accredited by the Council on Social Work Education or Master’s in Counseling or related field
- State of Ohio license as a Licensed Social Worker (LSW) or Counselor (LPC) required; LISW or LPCC preferred
